Establish whether productivity gains occur without compromising clinical outcomes

Establish whether the physician-productivity gains from NHS surgical hub adoption occur without adverse effects on clinical outcomes, including readmissions, complications, and patient-reported outcomes.

Background

The study measures cost-weighted elective HVLC output per unit of senior physician input but does not directly measure most dimensions of clinical quality. Consequently, the reported productivity gains cannot determine whether hospitals achieved higher output without compromising patient outcomes. The authors identify readmission, complication, and patient-reported outcomes as necessary measures for resolving this issue.

Whether the gain was achieved without cost to clinical outcomes cannot be settled with the measures used here, and readmission, complication, and patient-reported outcomes remain an important direction for further work.
